Medical Director, Value-Based Care

What You'll Do

As a clinical leader and transitional care management subject matter expert, the Medical Director, Central Clinical Services will provide clinical oversight and strategic direction for Strive's transitional care management (TCM), intensive care management (ICM), and utilization management (UM) functions. The Medical Director will ensure high-quality, cost-effective care delivery aligned with value-based care principles to patients in their most vulnerable states across the entire disease spectrum from CKD to ESRD, with the goal of optimizing outcomes and patient experience. This role will develop policies consistent with care management best practices and NCQA guidelines while driving Strive Health's mission of delivering exceptional kidney care. This role reports to the Chief Clinical Officer.

The Day to Day

  • Leads design, implementation, and ongoing effectiveness of TCM, ICM, and UM clinical programs in partnership with operations SVP of central clinical services and VP of care management in a dyad relationship.

  • Partners with Strive Regional Medical Directors and Specialty Medical Directors to ensure seamless transition of patients between central clinical to market clinical teams.

  • Leads clinically focused rounds, reviews complex clinical cases, facilitates problem-solving, and advocates for patient-level issues while providing daily feedback to individual clinicians on appropriate clinical management.

  • Acts as direct manager for Strive TCM NPs and serves on central corporate clinical committees, such as quality, compliance, or the Clinical Steering Committee as needed.

  • Develops educational content on care management best practices for patients transitioning from acute care settings and participates in onboarding of new hires.

  • Engages in various stages of the business development lifecycle, from initial pitches to creation of clinical workflows during implementation phase.

  • Provides consultation to product, tech, and operations teams as a subject matter expert with strong acumen for translating clinical program effectiveness to quantifiable clinical and financial ROI, including impact on total cost of care.

Minimum Qualifications

  • 6+ years experience working in cardiology and kidney care management, including experience with CHF, CKD, and ESKD populations. 

  • Current state-specific licensure required.

  • Current Drug Enforcement Administration (DEA) license required, or eligible to obtain within 90 days of hire.

  • Current BLS or CPR Certification required.

  • Board-certified in Nephrology, Internal Medicine, Cardiology, Family Medicine, Emergency Medicine, Geriatric Medicine, Hospice and Palliative Medicine, or another relevant specialty.

  • Efficient and reliable transportation, including an active driver’s license, allowing for the ability to travel to meet business needs.

  • Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms. 

Preferred Qualifications

  • Ability to innovatively think through and propose solutions for complex clinical challenges faced by patients with CHF, CKD, and ESKD.

  • Willingness to 'get into the weeds' with individual clinicians and cases daily to ensure consistency and quality of interventions.

  • Strong interpersonal and networking skills with effective approach to partnering with key operational and clinical stakeholders across regions.

  • Excellent communication, relationship development, results orientation, team building and decision-making abilities.

  • Demonstrated ability to excel in smaller entrepreneurial organizations with commitment to Strive Health's values and passion for advancing kidney care.

Annual Base Salary Range: $202,000 - $324,000
