Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Form Health provides high-quality expert care and leverages technology to enhance the patient experience. All Form Health patients work closely with their care team, which includes board certified physicians, advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. About the Role: The Licensing/Credentialing Specialist is responsible for efficiently managing licensing and credentialing processes company-wide. This role involves close collaboration with medical and dietary clinicians to ensure compliance with licensing activation and renewal requirements. A critical aspect of the position is the ability to complete new hire credentialing and privileging in adherence to NCQA standards, demanding meticulous attention to detail. Additionally, the specialist will assist in the process of credentialing applications for payer enrollment and will contribute to process improvements within the growing licensing and credentialing department.

What You Will Do:

  • Focusing on efficient licensing and credentialing task completion as requested by applicable third parties.

  • Supporting new hire credentialing and licensing documentation collection.

  • Complete new licensing and credentialing enrollment orders with 3rd party vendors.

  • Assisting with updates in various systems  as new licenses and credentials are granted.

  • Daily tracking and notification processes for license renewals.

  • Complete re-credentialing tasks

About You:

  • 2+ years of experience in credentialing or payer enrollment.

  • Experience in state licensing application and management preferred

  • Experience with scheduling platforms, EHR systems, or internal workflow tools (e.g., Jira, Asana, Salesforce).

  • Excellent organizational and communication skills.

  • High attention to detail, self-motivated and ability to manage multiple priorities.

  • Proficiency in Google Sheets/Excel; ability to analyze and spot schedule gaps or utilization issues.
